ERr 731 (standardized Siberian rhubarb extract) — Moderate evidence
ERr 731 is a specific standardized extract of Rheum rhaponticum (Siberian rhubarb root) that has been studied as a non-hormonal hot flash treatment in Europe for decades. A 2024 systematic review and meta-analysis in Heliyon (PMC) of eight clinical trials found ERr 731 produced significant reductions in the Menopause Rating Scale, particularly for hot flashes, sleep, and mood — with effect sizes larger than most other plant-based options.

Black cohosh is the most popular herbal menopause supplement and the most controversial. The evidence is genuinely divided. Vitamin D3 + calcium — Strong evidence
A 2020 meta-analysis of RCTs in postmenopausal women (PubMed) found that combined calcium and vitamin D significantly increased total bone mineral density, including at the lumbar spine and femoral neck — the sites that fracture and matter for quality of life.
