The phospholipid form matters because phospholipids are amphipathic — they have a water-loving and a fat-loving end. Evidence: Moderate.
The cleanest single answer comes from the 2024 network meta-analysis published in Heliyon, which pooled 60 randomized controlled trials and compared bioavailability across four endpoints: omega-3 index (O3i), area under the curve (AUC), time-to-peak (Tmax), and peak concentration (Cmax).
The headline finding: at low to moderate doses (under 2,000 mg of oil), krill oil's phospholipid form produced the highest AUC of any formulation tested, narrowly beating fish oil emulsions. But the picture flipped at higher doses. For raising the omega-3 index — the long-term blood marker that actually correlates with cardiovascular risk — fish oil at doses above 3,000 mg in re-esterified triglyceride or ethyl ester form was as effective or more effective than any krill oil dose studied.
A widely cited 2011 head-to-head bioavailability trial found that when EPA+DHA doses were matched, krill oil and fish oil produced essentially identical incorporation of EPA and DHA into plasma phospholipids over 4 weeks. The "krill absorbs better" advantage largely disappears once you control for the actual milligrams of EPA+DHA delivered.
What this means in practice
The phospholipid advantage is real but small, and it shows up most clearly at low doses. For severe hypertriglyceridemia specifically, a 2021 randomized clinical trial in JAMA Network Open tested a high-dose novel krill oil agent and found meaningful triglyceride reductions, but the dose used (4 g per day) is far higher than what is in any standard krill oil bottle and effectively nullifies the cost-per-gram advantage of fish oil.
Bottom line on lipids
If you have elevated triglycerides and your goal is to bring them down, both work. The clinically meaningful dose is 2–4 grams of EPA+DHA per day, which is achievable with a roughly $15–25/month fish oil concentrate or a $60–120/month krill oil regimen. Same outcome, very different cost.

Cardiovascular events and inflammation
Evidence: Limited for hard endpoints.
It is worth being honest here: the large randomized trials of omega-3s on actual cardiovascular events (heart attacks, strokes, deaths) — REDUCE-IT, STRENGTH, VITAL, ASCEND — were almost entirely conducted with fish-oil-derived EPA or EPA/DHA at high doses. For inflammation markers (CRP, IL-6, TNF-α), a 2025 comparative analysis in International Journal of Molecular Sciences found that krill oil's combination of phospholipid-bound EPA/DHA plus astaxanthin produced somewhat stronger antioxidant effects in cell and animal models than fish oil alone. Whether that mechanistic edge translates to clinically meaningful inflammation reduction in humans at real-world doses is still unsettled.

Krill oil naturally contains astaxanthin, a potent carotenoid antioxidant that is part of why krill oil resists oxidation better than fish oil and does not need added vitamin E for stability. The marketing leans heavily on this.
The catch: typical krill oil supplements provide 100–500 mcg of astaxanthin per serving. The clinical trials that show astaxanthin doing useful things — improving skin elasticity, reducing oxidative stress, improving exercise recovery — use 6,000–12,000 mcg (6–12 mg) per day. You would need to take 12–60 krill oil capsules to get a clinically active astaxanthin dose. Sustainability and contamination
This is the one place krill oil has a defensible advantage. Antarctic krill (Euphausia superba) is a very low-trophic-level organism that feeds on phytoplankton.
